Interdental Brush Replacement and Threader Reuse With Braces

TL;DR — Checked on August 23, 2026, exactly one of the seven manufacturer and professional-body pages in the audit table below puts a number of days on an interdental brush (GUM: "up to 7 days of use," with weekly replacement). TePe gives a condition instead of a date. Waterpik puts its orthodontic tip on a 3-month cycle. The AAO publishes nothing at all on replacing or cleaning an interdental tool; the ADA publishes no interval in days either, but does state that devices for cleaning between the teeth are not to be used more than once and are to be discarded after use. On floss threaders, two manufacturer pages call them disposable and a third says nothing about reuse — and no published study we could find has measured what happens when one is reused. Following the manufacturers' own maintenance schedules for brushes and pre-cut strands costs roughly $86 a year and discards about 417 plastic items per person per year.

Exactly one of the seven pages in the audit table below puts a number of days on an interdental brush: GUM's US Proxabrush page describes bristles treated with a hygienic solution for "up to 7 days of use," and tells users to rinse the brush after each use and replace it weekly. TePe's US care page gives a condition rather than a date — "Change the brush when the bristles become worn and start to collapse" — and Waterpik puts its orthodontic water flosser tip on a three-month cycle. The two US bodies most people would ask are further apart than they look: the American Association of Orthodontists publishes nothing on replacing or cleaning any interdental tool, while the American Dental Association publishes no interval in days but does treat between-the-teeth devices as single-use and states that they are to be discarded after use. What neither body publishes is an upkeep protocol for a tool that is meant to be kept. Most of what circulates beyond these pages is inference.

That matters more with fixed appliances than without, because braces multiply the hardware. A bracketed mouth typically runs a toothbrush, an interdental brush in one or two sizes, and either a threader or a pre-threaded strand for every gap — daily, for eighteen months or more. Yet the upkeep of that hardware is the part of the routine that almost nobody publishes on.

Method: what was checked, and how

On August 23, 2026 we opened eight pages — six from manufacturers, two from US professional bodies — and recorded two things from each: whether it states a replacement interval, and whether it states a cleaning instruction. Seven are tabulated below; the eighth, Waterpik's braces guide, is quoted in the threader section rather than tabulated, because it describes a product category instead of a specific item. Nothing was inferred; if a page was silent, that silence is what we logged.

Two findings are worth pulling out of that table. First, the only hard numbers in the whole set come from the companies selling the replacements — which is not a reason to dismiss them, but is a reason not to quote them as clinical thresholds. Second, the two brands that address an interdental brush at all do not answer the same question: GUM answers in days, TePe answers in bristle condition. A number and a condition are not interchangeable, and the gap between them is a fair measure of how firm the seven-day figure is.

What a rinsed brush is still carrying

The reuse question people actually have is blunter than the schedule question: is a brush that sat on the sink for a day disgusting? Two older studies are the closest published answers, and neither was done on braces wearers.

In a 2002 study of ten untreated chronic periodontitis patients, four interdental sites per person were cleaned professionally with a single interdental brush. Total anaerobic colony-forming units, plus P. gingivalis, F. nucleatum, and E. corrodens, correlated significantly with samples taken from the periodontal pockets themselves. The authors concluded that interdental brushes are contaminated "relatively easily" by these species in numbers comparable to their presence in the pockets.

The follow-on question — how long that survives a rinse — was measured in a 2001 study of twelve periodontitis patients. Brushes were rinsed with tap water and stored dry at room temperature. On toothbrushes without toothpaste, 10⁷ to 10⁸ colony-forming units were present immediately after rinsing, dropping insignificantly over the first 24 hours, with 10⁴ still culturable at 48 hours. No periodontopathogen remained detectable at 8 hours except F. nucleatum, and the proportion of viable bacteria fell from 50% to 30% over 48 hours. The paper reports "comparable results" for the interdental brushes.

Read carefully, those two studies say something more interesting than "gross" or "fine." Rinsing and drying does not sterilize anything, but the specific gum-disease species largely stopped being culturable within hours — while general bacterial load persisted for days. Both studies are small, both are more than twenty years old, both used professional cleaning, and neither involved fixed appliances. There is no equivalent measurement on a brush that has been worked under an archwire.

The toothpaste complication

The same 2001 paper found that detergent-containing toothpaste cut bacterial survival on brushes by roughly two orders of magnitude at baseline and more at later time points — which reads like an obvious upgrade to a rinse. It is not that simple. A 2024 laboratory simulation ran an ISO 2 interdental brush across dentin samples for 7,200 strokes in a simplified interdental space. Artificial saliva, a povidone-iodine solution, and chlorhexidine and fluoride gel slurries produced no measurable dentin loss; the three toothpastes produced linear surface damage of 12.6 to 26.5 µm, scaling with their abrasivity values. The authors' conclusion is that interdental products should be tailored and carefully instructed, and that combining interdental brushes with abrasive toothpastes is the misuse to avoid.

That is a laboratory rig, not a mouth, and it measured abrasion of tooth surface rather than cleanliness of the brush. It is why the laboratory result does not translate directly into a recommendation about putting toothpaste on an interdental brush — a question that belongs to an orthodontist or hygienist who can see the appliance rather than to an article.

Threaders: called disposable, never measured

Floss threaders are where the published record thins out to almost nothing. Colgate's page calls them disposable and instructs the reader to discard the threader after use. Waterpik's braces guide describes them as "disposable floss threaders." GUM's own product page for the Eez-Thru threader describes the flexible nylon loop and the appliances it is meant for, but does not state whether it may be used twice.

The ADA's FAQ is the closest thing to an authority position, and it deserves precision rather than either amplification or trimming. Under the question "Can I rinse and reuse floss?" the answer reads in full: "The ADA does not recommend using devices to clean between the teeth more than once. Used floss might fray. Any of these products may become worn, reducing effectiveness or may deposit bacteria from the previous use back into the mouth. Discard after use." That is a replacement rule — the strictest one available, single use — and it comes with a stated reason. What is genuinely unresolved in the text is whether a threader is one of the "devices": a threader carries the floss under the wire but does not itself do the cleaning, and the page never addresses that distinction. We could not find a published study, of any design, that has measured contamination, breakage, or cleaning performance of a reused floss threader. That absence is the honest answer, and it is worth more than a confident one.

Our best hypothesis, offered as a hypothesis rather than a finding, is mechanical rather than microbial: a nylon loop that is pulled through a tight contact and around an archwire is being fatigued. Stem durability testing of five commercial interdental brushes bent them 90 degrees under a controlled 500 g load and found a significant difference between geometries — L-shaped stems showed lower fracture resistance than straight ones. That documents a difference between shapes rather than a mechanism, and extending it from wire stems to nylon loops is an extrapolation on top of that. Both should be read as such.

What the published schedule costs, and what it throws away

Taking the manufacturers at their word produces a number nobody publishes, so here is the arithmetic with its assumptions in the open. One interdental brush size, replaced weekly, is 52 brushes a year. A 20-pack of TePe Original ISO 3 brushes was $21.99 at Target in the US on August 23, 2026, which is $1.10 per brush and $57.17 a year — doubling to roughly $114 for anyone fitted with two sizes. A Waterpik orthodontic tip replaced quarterly is four tips a year; at $14.99 for two tips on Waterpik's own US store, that is about $30 a year. One pre-cut threader strand a day is 365 strands; Oral-B Super Floss was $3.99 for 50 pre-cut strands at Target, about 8 cents a strand and $29.13 a year. Brushes and strands together come to $86.30 a year; adding the water flosser tips brings it to roughly $116.

Added up, the manufacturers' own maintenance schedules discard roughly 417 plastic items per person per year — 52 brushes and 365 strands — before counting caps, sleeves, and cartons. The brushes are the harder half: nylon bristles on nylon-coated metal wire in a plastic handle is a bonded mixture of three materials, which is effectively unsortable at end of life. Where the published guidance stops

None of the numbers above came from a clinical trial. No randomized study has compared replacement intervals, and the systematic review of interdental brushes in fixed-appliance patients — six heterogeneous trials, 442 patients — describes only low-certainty, tentative indications of benefit from the brushes themselves, let alone from any upkeep regimen. The broader Cochrane review of 35 trials and 3,929 adults grades the whole interdental evidence base as low to very low certainty and notes the effects observed may not be clinically important.

So the defensible position is narrow. Manufacturers state a cadence; the microbiology says a rinse reduces rather than eliminates; the professional bodies publish no upkeep protocol at all, one of them saying nothing and the other saying discard rather than reuse; and no published study we could find has tested a threader twice. Anything beyond that — bleeding, soreness, a bristle that has bent the wrong way, a brush size that no longer fits, a loose bracket or a distorted archwire — is a matter for the treating dentist or orthodontist, who can see the appliance. A replacement schedule is not a diagnostic tool and cannot substitute for one.
